Individual and Couples Therapy
We currently accept Blue Cross Blue Shield, TriCare, Medicaid (21 and under), and Self-Pay (see sliding scale below) for therapy sessions.
We do not bill secondary insurance without prior approval.

Sliding Scale Rates

50,000 annually or less $50
50,000- 60,000 annually $60
60,000-70,000 annually $70
70,000-80,000 annually $80
80,000 and 90,000 $100 a session
100,000 plus Full Price Session $150

Testing Prices
All testing is self pay; we do not bill insurance for our testing at this time.
Attention Deficit Disorder (ADHD) Testing: $200; $100 non-refundable deposit required to schedule
Autism Assessment: $550; $150 non-refundable deposit required to schedule
MMPI: $250; $100 non-refundable deposit required to schedule
Full Battery: $1000+; $150 non-refundable deposit required to schedule
An intake will be completed prior to testing and after the intake the full price of the testing will be discussed.

New Patient Forms
Below are the required intake forms for all New Patient appointments and testing appointments.
Please note: all paperwork must be submitted 24 hours before prior to your intake appointment, including any and all custody paperwork as applicable.
